Who is responsible for your information

What we collect
When you fill out our request form, we collect your first name, last name, address, phone number, the role of the professional you need, the number of hours wanted per week, the number of weeks and the description of the health condition you provide.
Health information. The description of a health condition is sensitive personal information. We collect it only to assess the service required and to follow up. Please share only what is necessary for your request, and never your health insurance number or a full medical record.
If you accept measurement cookies, we also collect browsing data through Google Analytics and Google Ads. If you decline, none of these cookies are set.
We also use Matomo, an audience statistics tool configured to use no cookies at all. It counts pages viewed and clicks on our contact details, without storing a file on your device, without identifying you and without keeping your full IP address. It honours your browser’s Do Not Track signal. Because it stores nothing, it works whether or not you accept the banner.

We do not sell or rent your information, and we do not share it with third parties for commercial purposes.
Where your information is stored
Requests sent through the form are routed to an automation server located in the United States, then emailed to our team. Google Analytics and Google Ads process browsing data on servers outside Canada.
Matomo audience statistics are hosted on a server located in Boston, United States. They contain no information that could identify you.
We tell you this because Quebec law requires that you know when your information leaves the province.
How long we keep it
Form requests are kept for three years, then deleted. Client files are kept according to the rules applicable to health services.
Your rights
You may at any time ask us to access, correct or delete your information, or to withdraw your consent. Write to [email protected]. We answer within 30 days.
If our answer does not satisfy you, you may file a complaint with the Commission d’accès à l’information du Québec.
Cookies
On your first visit, a banner asks you to accept or decline measurement and advertising cookies. Until you choose, no such cookie is set. To change your choice, clear this site’s data in your browser.
Our Matomo audience statistics set no cookie, so they are not covered by this choice.
